TriState Health will host their Annual Free Sports
Physical Clinic on Wednesday, June, 26, from 4:00PM-7:00PM, at TriState Family Practice & Internal
Medicine, located at 1119 Highland Avenue, Suite 10, Clarkston, WA. There is no appointment
necessary. The clinic is open to anyone seeking a sports physical for their children.

TriState Health asks that the child or accompanying parent bring the required physical form from the
school that they attend or will be attending. Please include demographics (name, birthdate, and
address) and complete before arriving. General forms will also be available upon arrival. Children under
the age of 18 must be accompanied by a parent or guardian.
